Hammond Home Health provides tools to help users monitor wellness data such as heart rate, steps, blood glucose, and blood pressure. Data may come from Apple HealthKit, Apple Watch, wearable devices, or user input.

Not a Medical Device:
Hammond Home Health is not a medical device and does not provide medical advice. The service is intended for informational and wellness purposes only and is not intended to diagnose, treat, cure, or prevent disease. Always consult a qualified healthcare
professional.

Health Data Accuracy:
Data may originate from third‑party devices or Apple HealthKit. Hammond Home Health cannot guarantee accuracy of data generated by external devices.

HealthKit Integration:
If permission is granted, the app may read specific categories of health data from Apple HealthKit. HealthKit data is never sold and is never used for advertising.

Remote Monitoring:
Users may optionally share health data with caregivers or healthcare providers. Sharing occurs only with explicit user consent.

User Responsibilities:
Users must provide accurate information, maintain device security, and use the app only for lawful purposes.

Limitation of Liability:
Hammond Home Health is not liable for decisions made based on app data, device inaccuracies, or service interruptions.

  • Health Data (heart rate, steps, blood glucose, blood pressure, activity metrics)

HealthKit Data Use:
HealthKit data is accessed only with user permission and is used solely for health monitoring features. HealthKit data is not used for advertising and is not sold to third parties.

How We Use Information:

  • Display health metrics in the app
  • Enable remote monitoring features
  • Improve application performance
  • Provide technical support

 

Data Sharing:
Information may only be shared with user consent, authorized caregivers or healthcare providers, or when required by law.

Data Security:
We use encryption, authentication controls, and secure infrastructure to protect user data.

User Rights:
Users may request access, correction, or deletion of their data and may revoke HealthKit
permissions in iOS settings.

Outside of work, Chelsea has a big heart for animals, fostering dogs in need of loving homes. She also treasures quality time with her husband and daughter, making every moment count. Chelsea truly embodies the values of compassion, dedication, and service that drive our organization forward.

an HR Manager with extensive experience in patient care, Amy specializes in supporting Medicaid patients and Private Pay patients through compassionate and personalized services. Outside of work, I love spending quality time with my family, helps other and can be seen almost every day at 7-11 to indulge in her favorite treat- Coke Slurpees!

Scott Hammond is a proud United States Coast Guard Veteran, having served with honor and dedication. Scott then became an Emergency Room Nurse, and has done so for almost 25 years. He has been happily married for over 22 years and is a loving father to four children. Scott has been a resident of Frisco, Texas, for 21 years, where he has built a strong connection with his community.

Outside of his professional and family life, Scott enjoys spending his free time fishing and exploring his love for cooking in the kitchen. He values family time above all, cherishing moments spent with his loved ones.
