Hi, I’m Fred Hammond, CEO and Founder of Hammond Home Health Care, LLC. At Hammond Home Health Care, we are driven by a simple yet profound philosophy: Treat others as we would like to be treated.

This guiding principle, instilled in me by my father, forms the foundation of everything we do. After a successful 30-year career in executive leadership with Fortune 500 companies, I discovered a deep passion for listening to people, identifying their needs, and developing solutions that enrich their lives.

In 2004, my father was diagnosed with a rare form of cancer called Multiple Myeloma. During that time, we were met with a lack of empathy and inadequate answers. It was then that I realized just how incredibly challenging, sterile, and lonely it could be for families desperately searching for guidance. Finding accurate information, understanding insurance policies, and making informed decisions often felt overwhelming—especially during moments of vulnerability.

It shouldn’t be this way. Families deserve trustworthy guidance and clarity, delivered with empathy and expertise. At Hammond Home Health Care, we are committed to simplifying this process and being the trusted partner you can rely on.

We also recognize that the home is the most comforting place to receive care. It’s where healing feels natural, memories are cherished, and independence is preserved. Chelsea Stine is our dedicated Field Operations Representative and was proudly our very first employee. With a deep commitment to excellence, Chelsea ensures that all Personal Attendant Services (PAS) on-boarding processes are seamless and that care is delivered at the highest standard. Her leadership and dedication set the tone for the exceptional service we provide.

Outside of work, Chelsea has a big heart for animals, fostering dogs in need of loving homes. She also treasures quality time with her husband and daughter, making every moment count. Chelsea truly embodies the values of compassion, dedication, and service that drive our organization forward.

Amy Kennedy

Personal Attendant Service Manager

an HR Manager with extensive experience in patient care, Amy specializes in supporting Medicaid patients and Private Pay patients through compassionate and personalized services. She ensures our team delivers top-notch care that enhances the lives of our patients every day. She is deeply passionate about creating a workplace culture that fosters growth, empathy, and exceptional service delivery. Outside of work, I love spending quality time with my family, helps other and can be seen almost every day at 7-11 to indulge in her favorite treat- Coke Slurpees!

Community Case Manager

Community Case Manager has passion for serving others shines through her career journey. With 22 years of experience at Dallas ISD, Ana transitioned into home healthcare, where she found true fulfillment in caregiving. Her heart for helping others is evident in her role as a Community Case Manager. Ana’s approach is hands-on—she enjoys meeting new people and discovering how she can be a blessing in their lives. Her dedication to serving the community reflects her deep commitment to making a positive impact on those around her.

I am a devoted mother of two and grandmother of one, with a deep faith that keeps me focused and grounded. Spending quality time with my family is a top priority, and I cherish every moment with my loved ones.

Patient Engagement Specialist

A dedicated patient care coordinator and medical assistant with 5 years of experience, and a loving mother of two children. I specialize in connecting patients to medical resources and support systems, while facilitating communication between patients and healthcare providers. I also monitor patients’ progress toward their healthcare goals to ensure comprehensive care.

I’m an Acute Care Marketing Manager dedicated to helping people understand healthcare benefits. Scott Hammond is a proud United States Coast Guard Veteran, having served with honor and dedication. Scott then became an Emergency Room Nurse, and has done so for almost 25 years. He has been happily married for over 22 years and is a loving father to four children. Scott has been a resident of Frisco, Texas, for 21 years, where he has built a strong connection with his community.

Outside of his professional and family life, Scott enjoys spending his free time fishing and exploring his love for cooking in the kitchen. He values family time above all, cherishing moments spent with his loved ones.

Beyond his professional achievements, Fred treasures quality time with his two grandsons, who lovingly call him “Papa Ham.” A devoted tennis enthusiast and avid traveler, Fred embraces life with passion, energy, and a sense of purpose.
